If you are already a patient of Dr. Gross and have a change in your personal information, please print and fill out any changes and bring them with you to your appointment.

Along with your forms you may also need the following information:
* A current photo ID
* Current insurance card
* X-rays (if taken previously) relating to your current condition
* Your co-pay listed on your insurance card
* Your referral from your Primary Care Physician if needed

Some insurance plans may have restrictions to hospitals, referrals, physicians, as well as the type and extent of treatment. Changes in hospitals, participating physicians, labs, and ancillary service providers occur continuously. While we make every effort to keep abreast of these modifications and relay them to you, it is ultimately the patients responsibility to make certain your program approves of the choice of hospital, physician, or laboratory where you have been referred. We recommend careful review of your plan’s benefits and suggest contacting your program’s patient relations representative for explanations.
Our fees are within the customary range for this area and reflect the level of care you will receive and the highest level of training by the physician in the group. Payment will be requested at the time of all services that are non-covered or determined to be the patient’s responsibility, including co-payments.
Charges for all office visits are payable at the time of your visit unless other arrangements prior to your appointment have been made.
Certain insurance plans require prior authorization from a primary care provider in order for you to be seen. If your plan requires a referral , you MUST bring your referral in order to receive treatment.
